You can now request a repeat prescription for your pet's medication using the form below. Please include your pet's name, the quantity and strength of any medication you are requesting and specify which surgery you would like to collect the medication from. Please also include a phone number in case we need to contact you regarding the prescription.



Prescription requests placed Mon - Thurs will be available 48 hours later. Prescription requests placed on a Friday will be available by the following Tuesday.
